Chiari Malformation

Chiari malformation is a structural anomaly where the lower part of the brain- specifically the cerebellar tonsils- extends downward through the opening at the base of the skull (the foramen magnum) and into the upper spinal canal. This displacement creates a mechanical mismatch that can obstruct the natural, rhythmic flow of cerebrospinal fluid (CSF) between the cranial cavity and the spinal channel, leading to increased pressure and structural complications.

Many individuals live with mild Chiari anatomy without ever experiencing symptoms, often discovering the condition incidentally on an MRI. However, when the structural crowding becomes severe, it can manifest as a distinct, pressure-like headache at the back of the head that worsens with coughing or straining. It can also cause balance disruption, dizziness, fine motor challenges, or the development of a syrinx- a fluid-filled cavity within the spinal cord itself.

If a Chiari malformation is causing progressive neurological deficits or interfering with CSF flow, our focus shifts to creating structural breathing room. This is achieved through a precise posterior fossa decompression. During this procedure, a small section of bone at the back of the skull is carefully removed, and the protective lining of the brain (the dura) is widened with a customized patch graft. This intervention restores natural fluid dynamics, relieves brainstem compression, and provides long-term structural stability.
